In this vital role, you will provide high-quality, personalized care to our residents and patients, ensuring their comfort, safety, and well-being. Your energetic approach will help create a warm and supportive environment where individuals feel valued and cared for. Whether working in long-term care, acute hospital settings, or community-based programs, your expertise will make a meaningful difference every day. This position offers an exciting opportunity to apply your diverse skills in a dynamic healthcare environment committed to excellence. Responsibilities Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using tools like Hoyer lifts when necessary. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely documentation within electronic health record (EHR) systems like Epic or Cerner. Support patients with specialized needs such as Alzheimer's or dementia care, memory care units, or behavioral health requirements with patience and understanding. Administer medications and treatments as prescribed under HIPAA guidelines and maintain precise medical documentation of all patient interactions. Assist with medical procedures including catheter care, phlebotomy tasks, aseptic techniques, and basic life support during emergencies. Maintain a safe environment by adhering to infection control protocols, using proper PPE in infectious disease or Level I trauma center scenarios. Support patients with mobility aids such as Hoyer lifts or transfer devices while practicing proper body mechanics and safety procedures. Qualifications Valid CNA certification with current licensure in the state of practice. Experience working in diverse healthcare environments including long-term care facilities, hospitals (ICU, trauma centers), assisted living communities, or outpatient clinics. Proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for medical documentation and patient records management. Knowledge of medical terminology, anatomy, physiology, and aseptic techniques essential for safe patient care delivery. Experience with specialized care areas such as Alzheimer's/dementia/memory care or behavioral health is highly desirable. Ability to perform vital signs measurement accurately and respond effectively in urgent or acute care situations including Level II trauma center experience or emergency medicine settings. Strong caregiving skills combined with excellent communication abilities to work effectively with individuals of all ages—including pediatrics—and those with disabilities or developmental challenges. Physical stamina to stand for extended periods and assist with patient transfers using appropriate equipment like Hoyer lifts or transfer boards. Join us to be part of a vibrant team dedicated to elevating patient care through compassion, expertise, and teamwork!
