Duties Assist patients with daily activities such as bathing, dressing, feeding, and mobility to promote independence and well-being. Monitor vital signs including blood pressure, pulse, temperature, and respiration rates; accurately document findings in electronic health records (EHR/EHR systems). Support medical procedures such as catheter care, wound dressing changes, phlebotomy (blood draws), and medication administration under supervision. Maintain a safe environment by utilizing proper manual handling techniques, operating patient transfer methods like Hoyer lifts, and adhering to safe lifting practices. Observe and report changes in patient condition promptly to nursing staff; communicate effectively with patients, families, and interdisciplinary team members. Follow infection control protocols using aseptic techniques; ensure compliance with HIPAA regulations to maintain patient confidentiality at all times. Assist with patient transfers using appropriate methods; support mobility needs for patients with complex conditions such as dementia or post-acute care requirements. Qualifications Current certification as a Nursing Assistant (CNA) recognized by the state or relevant licensing authority. Experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities, or those requiring specialized memory or Alzheimer's care. Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks; experience documenting medical records accurately. Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS/CPR). Prior experience in long-term care facilities, hospitals (including ICU or trauma centers), hospice or home & community care environments is highly desirable. Ability to perform heavy lifting safely using proper techniques; comfortable with manual handling equipment like Hoyer lifts. Strong caregiver communication skills; capable of working effectively within multidisciplinary teams in fast-paced settings. Additional skills such as medical-surgical procedures, infection control practices, or emergency medicine experience are a plus.
