In this vital role, you will provide direct patient care and support services in homes and in individual communities. You will assist with daily activities, and support other staff in delivering high-quality care across diverse settings. Duties Deliver compassionate patient care by assisting with activities of daily living such as bathing, dressing, feeding, mobility support and transfer assistance. Monitor vital signs including blood pressure, pulseand temperature to track patient health status accurately Maintain patient confidentiality and adhere to HIPAA regulations while providing services and documenting patient information Support individuals with medication reminders Assist with specialized care needs including Alzheimer's care, dementia care, memory care, behavioral health support, and working with individuals with developmental disabilities or physical disabilities Use manual handling techniques safely during patient transfers to prevent injury for both patients and staff Collaborate effectively with management and other staff to coordinate patient care plans Participate in infection control practices including infectious disease care protocols to maintain a sterile environment Transporting clients to doctors appointments, and providing support services when out in the community Experience Prior experience working as a CNA or in similar caregiving roles within hospital, long-term care facilities, assisted living communities, or home & community care settings is highly preferred Demonstrated knowledge of anatomy and physiology relevant to patient care across specialties such as geriatrics or behavioral health Experience working with seniors and individuals with intellectual or physical disabilities Experience providing personal care, transportation, in home and community support Familiarity with specialized areas such as senior care, intellectual and physical disabilities enhances your ability to adapt quickly Certification in Basic Life Support (BLS) or CPR is required; additional certifications like medication administration or phlebotomy are a plus Strong communication skills paired with caregiving expertise enable you to connect effectively with individuals of all ages while maintaining professionalism and empathy Join us to be part of a vibrant team committed to delivering exceptional patient-centered care.
