Multiple Shifts Available 8pm-8am 8am-8pm Start ASAP Overview Join our dedicated healthcare team as a Home Health Aide & Personal Care Assistant, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this role, you will provide essential support to individuals in their homes, helping them maintain independence, comfort, and dignity. You will be the vital link between professional healthcare services and personalized home care, ensuring each client receives the highest quality assistance tailored to their unique needs. This paid position offers an exciting opportunity to grow your caregiving skills while making a positive impact in the community. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, managing medication administration, and reporting changes in condition accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide specialized care for clients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavioral support. Support clients with medical procedures such as catheter care, wound dressing, and basic first aid or CPR when necessary. Maintain strict adherence to home healthcare regulations and HIPAA guidelines by safeguarding patient confidentiality and medical records. Prepare nutritious meals aligned with dietary restrictions, assist with light housekeeping tasks like cleaning and laundry to ensure a safe living environment. Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met comprehensively. Qualifications Proven experience working with individuals with disabilities, developmental disabilities, or in geriatrics; assisted living or nursing home experience is highly valued. Knowledge of medical terminology, patient transfer methods, and home health care assistance techniques. Certification in First Aid and Basic Life Support (CPR), with additional training in hospice or end-of-life care preferred. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or patient transfer devices. Strong communication skills for client interaction and team collaboration; experience working with geriatric patients is advantageous. Understanding of home health agency operations and home & community care standards; experience working with individuals with dementia or Alzheimer's is a plus.
