• Deliver compassionate, patient-centered care in a hospice setting
• Collaborate with an interdisciplinary team focused on comfort, dignity, and quality of life
• Provide hands-on care assisting with activities of daily living (ADLs)
• Observe and report changes in patient condition to support safety and independence
• Educate and support patients and caregivers by reinforcing care plans and promoting healthy routines
• Respond promptly to patient needs and communicate effectively with the care team
• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Work independently while having team support
• Travel to patient homes within assigned territory Schedule and Shift Details
• 2-3 days per week, Monday through Friday, 8am to 5pm Travel
