Job Description

This vital role involves providing hands-on support, ensuring patient comfort, and assisting with daily activities in environments ranging from long-term care facilities to acute hospital units. Responsibilities Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts and transfer methods. Monitor vital signs and observe patient conditions to promptly report changes to nursing staff. Maintain accurate medical documentation and update electronic health records (EHR/Epic systems) in compliance with HIPAA regulations to ensure patient confidentiality. Support infection control protocols through aseptic techniques and proper hand hygiene practices during patient interactions. Assist with medical procedures including catheter care, medication administration under supervision, and basic life support (BLS) when necessary. Facilitate safe patient transfers using appropriate manual handling techniques and equipment. Communicate effectively with patients, families, and interdisciplinary teams to promote understanding and comfort. Qualifications Valid certification as a Nursing Assistant (CNA) in accordance with state requirements. Experience working with diverse populations such as geriatric patients, individuals with developmental disabilities, or those requiring dementia or Alzheimer's care. Familiarity with EMR/EHR systems like Cerner or eClinicalWorks is preferred; strong computer literacy is a plus. Knowledge of medical terminology, anatomy, physiology, and basic nursing procedures including vital signs measurement and CPR certification. Prior experience in settings such as long-term care facilities, hospitals (including ICU or trauma centers), hospice, or outpatient clinics is highly desirable. Ability to lift heavy loads safely using proper lifting practices and assistive devices like Hoyer lifts. Excellent caregiver communication skills with a focus on compassionate patient service.
