In this role, you will assist with daily living activities, monitor patient health, and support overall well-being in a variety of care settings including assisted living, nursing homes, and home healthcare environments. Your energetic and attentive approach will make a meaningful difference in the lives of those you serve, ensuring they receive respectful and personalized care every day. Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support Assist with activities of daily living (ADLs) such as eating, toileting, and transferring using patient transfer methods and equipment like Hoyer lifts Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems Support patients with memory care needs, dementia or Alzheimer's care, and end-of-life care with patience and compassion Maintain patient confidentiality in accordance with HIPAA regulations while accurately recording medical documentation and records Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment Communicate effectively with patients, families, and healthcare teams to ensure coordinated care delivery Requirements Valid certification as a Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA) Experience working with geriatric patients or individuals with developmental disabilities is highly preferred Knowledge of medical terminology, catheter care, and infection control practices including first aid and CPR certification Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Strong understanding of home health care regulations and compliance standards including HIPAA Excellent patient communication skills with a caring attitude towards diverse populations including those requiring hospice or memory care services Ability to work independently or as part of a team in fast-paced environments such as assisted living facilities or home healthcare settings Join us to make a positive impact through dedicated caregiving!
