Job Summary Join our dynamic healthcare team as a Contract CNA (Certified Nursing Assistant) or LPN (Licensed Practical Nurse), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, high-quality nursing support across various healthcare settings, including hospitals, long-term care facilities, assisted living communities, and home care environments. Your dedication will help ensure seamless patient assessments, medication administration, and vital sign monitoring while adhering to strict safety and privacy standards. This position offers flexible scheduling options and the opportunity to work in diverse clinical environments, making a meaningful difference in patients' lives every day. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) Point Click Care. Administer medications and injections accurately following prescribed protocols while maintaining compliance with HIPAA regulations and infection control standards. Assist with medical procedures such as IV infusions, catheterization, wound care, tube feeding, and sterile dressing changes utilizing aseptic techniques. Support patients with activities of daily living (ADLs), including mobility assistance with Hoyer lifts, personal hygiene, and nutrition management. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with complex needs like dementia, Alzheimer's disease, or developmental disabilities. Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks, ensuring accuracy for medical records and billing purposes. Qualifications Valid CNA certification or LPN license in good standing; additional certifications such as Basic Life Support (BLS) are required. Demonstrated experience working 6 mos or more in Skill Nursing setting. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to support accurate patient assessment and documentation. Ability to work effectively with individuals with disabilities or cognitive impairments such as dementia or memory care needs. Excellent communication skills coupled with a compassionate approach to patient-centered care; ability to handle high-pressure situations confidently.
