Responsibilities include:

Providing direct care to ambulatory and non-ambulatory residents Assisting with bathing, dressing, grooming, feeding, toileting, and mobility Observing residents and reporting changes in physical, emotional, or behavioral conditions to licensed nursing staff Supporting prescribed rehabilitative, therapeutic, and recreational activities Promoting resident independence while ensuring safety and comfort Maintaining clean, organized, and safe resident living areas Accurately documenting care provided Collaborating with nurses, physicians, and the interdisciplinary healthcare team Following established clinical, infection control, and safety procedures Every interaction matters. Preferred qualifications include:

Current Pennsylvania Certified Nursing Assistant (CNA) Certification Pennsylvania Department of Health Nurse Aide Registry certification for Long-Term Care High school diploma or GED One year or more of paid CNA experience providing direct patient care (strongly preferred) Experience working with geriatric, medically complex, or intellectually/developmentally disabled individuals is highly desirable Basic computer skills, including Microsoft Office Strong communication, observation, and documentation skills Excellent attendance, professionalism, and dependability Ability to work independently while collaborating effectively with an interdisciplinary healthcare team Ability to successfully complete all required background clearances and onboarding requirements Important Information Selinsgrove State Center is a secure, state-operated healthcare facility dedicated to providing exceptional care in a structured environment.
