POSITION SUMMARY
To meet the physical and psychosocial needs of the residents based on the plan of care and instruction of staff and/or nurse.
SPECIFIC DUTIES AND RESPONSIBILITIES
Report at start of shift with pocket care plans/worksheet to receive shift report. At the end of the shift give the report to the oncoming shift. Answer call bells in a timely manner. Prioritize care with coworkers in assigned neighborhood based on resident needs. Regular punctual attendance is an essential function. Become familiar with each resident RASP (Resident Assessment Support Plan). Review and update residents RASP document as needed. Participate in acquiring needed information to keep RASP accurate when information is requested. Assist residents with the activities of daily living including but not limited to bathing, dressing, mouth care, toileting, and transfers. Responsible for appropriate documentation of activities. Assist residents with all IADL'S according to their needs dictated through the residents RASP Inform nurses of any findings or observations while caring for residents i.e., skin tears, bruises, accidents, incidents, change in status with dignity and in appropriate forum. Ensure competency by completing the state mandated 12 hours of inservices yearly. Ensure competency in state mandated additional 6 hours of dementia training. Awareness of safety hazards by maintaining a clean, neat and clutter free environment. Ensure residents rooms are safe for their ambulatory device and are clean. Maintain friendly, professional attitude with residents, visitors and coworkers, and other disciplines. Understand and use the Gait Belt if needed. Attend Resident Care Conference for neighborhood, meetings and inservices as required. Always maintain HIPPA regulations and residents' dignity Assist with vital signs. Become familiar with all Personal Care policies and procedures per the Policy and Procedure binder. Maintain current CPR/FA certification and state certification for all Certified Nursing Assistants in accordance with State requirements. Report suspected abuse, neglect or other resident care issues. Assist with fire or other emergencies as needed. Participate in fire drills Complete all other duties as assigned.
Qualifications:
Required:
Current Certified Nursing Assistant (CNA) certification in the Commonwealth of Pennsylvania. Current CPR certification (or ability to obtain). Excellent communication and interpersonal skills. Ability to provide compassionate, resident-centered care. Ability to work effectively as part of a multidisciplinary team.
Preferred:
Previous experience in a senior living, long-term care, or healthcare setting. Experience caring for older adults. Experience with electronic health records.
