In this vital role, you will provide personalized, respectful, and high-quality care to patients in their final stages of life, ensuring comfort and dignity. Your energy and empathy will help create a supportive environment for patients and their families during challenging times. This position offers an opportunity to make a meaningful difference while working in a dynamic healthcare setting that values professionalism and teamwork. Duties Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support. Provide specialized hospice care that emphasizes comfort measures, pain management, and emotional support for terminally ill patients. Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems. Assist with transfers and positioning to prevent bedsores and promote patient comfort in accordance with safety protocols. Engage with patients experiencing dementia or memory care needs, applying techniques suitable for Alzheimer's and other cognitive impairments. Qualifications Valid certification as a Certified Nursing Assistant (CNA) with current state licensure. Dependable transportation Experience working in hospice or palliative care environments is highly preferred. Demonstrated knowledge of medical terminology, anatomy, physiology, and aseptic techniques. Experience with EMR/EHR systems for accurate documentation. Prior work in long-term care facilities such as nursing homes, assisted living, or home & community care settings preferred. Strong caregiving skills with an emphasis on geriatrics, behavioral health, dementia care, and working with individuals with disabilities or developmental challenges. Excellent communication skills combined with the physical stamina required for patient service tasks such as lifting and transferring using Hoyer lifts or other assistive devices.
