Job Description
Overview Join our compassionate healthcare team as a PRN Certified Nursing Assistant (CNA) Field Staff and make a meaningful difference in the lives of patients across the home health setting. This flexible, per-diem role offers you the opportunity to provide essential patient care, support clinical teams, and promote comfort and safety for individuals of all ages and health conditions. Responsibilities Assist patients with activities of daily living such as bathing, dressing, feeding, and mobility to ensure comfort and dignity. Monitor vital signs including blood pressure, pulse, temperature, and respiration; accurately document findings using Electronic Health Records (EHR) systems. Maintain patient confidentiality in accordance with HIPAA regulations while managing medical documentation and records efficiently. Collaborate effectively with your fellow staff. Requirements Valid CNA certification with current state licensure; CPR certification required. Experience working in a Home health setting. Atleast a year in the field. Familiarity with EMR for accurate documentation. HCHB-Pointcare is used by this agency. Knowledge of medical terminology, vital signs assessment, and infection control procedures. Prior experience caring for geriatric patients or individuals with complex health needs such as dementia or behavioral health conditions is highly desirable. Strong communication skills to effectively engage with patients of all ages and collaborate within multidisciplinary teams.
