The ideal candidate will possess a strong foundation in patient care, with experience working with diverse populations including geriatric, pediatric, and individuals with developmental disabilities. This role offers an opportunity to make a meaningful difference in patients' lives by delivering high-quality support in hospitals, long-term care facilities, assisted living communities, and home care environments. The successful candidate will demonstrate excellent communication skills, attention to detail, and a commitment to maintaining the highest standards of safety and confidentiality. Responsibilities Assist patients with activities of daily living and transportation to and from Dr appts. Monitor vital signs including blood pressure, pulse, respiration, and temperature; accurately document findings within electronic health record (EHR) systems while maintaining HIPAA compliance. Support medication administration under supervision and ensure proper handling of medical supplies such as catheters and DME (Durable Medical Equipment). Collaborate effectively with staff and other healthcare professionals to ensure seamless patient service across inpatient, outpatient, or home settings. Assist with patient transfers using safe lifting practices and manual handling techniques; utilize equipment such as Hoyer lifts when necessary. Maintain accurate medical documentation of patient interactions, treatments, and observations using EMR/EHR systems for comprehensive record-keeping. Demonstrate proficiency in emergency procedures including CPR and basic life support (BLS), especially within trauma or ICU environments. Communicate compassionately with patients and their families to provide emotional support and education about care plans. Qualifications Valid CNA certification or HHA license required; additional certifications such as BLS/CPR are highly preferred. Experience working in diverse healthcare environments including hospitals (medical-surgical units, ICU, trauma centers), long-term care facilities, assisted living communities, or home health settings. Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks; proficiency in medical documentation is essential. Prior experience with specialized care areas such as geriatrics, pediatrics, infectious disease management, or hospice is advantageous. Knowledge of medical terminology, anatomy, physiology, and aseptic techniques necessary for safe patient care delivery. Ability to perform heavy lifting safely while adhering to safe lifting practices; physically capable of manual handling tasks required for patient transfers. Strong caregiver communication skills coupled with a respectful approach towards patients of all ages and backgrounds. Demonstrated ability to work independently as well as part of a multidisciplinary team in fast-paced environments such as Level I Trauma Centers or teaching hospitals.
