Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs Observe patient condition and report changes promptly to RN Supervisor Support patients with specialized needs such as Alzheimer's care, dementia care, and working with individuals with developmental disabilities or disabilities Maintain accurate documentation for clients. Prepare nutritious meals and assist with meal planning while ensuring dietary restrictions are followed; perform light cleaning and laundry to maintain a safe environment Use medical terminology appropriately during patient interactions and documentation to ensure clear communication among healthcare teams Skills Strong patient observation skills to detect changes in condition or behavior promptly Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience Knowledge of assisting with ADLs, medication reminders Ability to operate medical equipment such as Hoyer lifts safely and effectively CPR certification is required; additional certifications like HIPAA compliance are highly valued Excellent caregiving skills combined with compassion for patients' physical and emotional well-being Join us in delivering exceptional home health services that prioritize dignity, independence, and compassionate care.
