Salary:
$16-17.00 /hour starting out or based on experience.

Description of Position:
Provides personal care for activities of daily living to the residents living in Canterfield. Promotes resident health in the six areas of wellness: physical, emotional, social, spiritual, intellectual, and vocational Communicates with other departments and shifts as necessary to ensure needs are being met
Responsibilities:
1 Assist with activities of daily living (as listed but not limited to showering, toileting, personal hygiene and grooming, pivot transfers, activities, vital signs, etc.) as directed on the Resident ADL & Wellness Care Plan. 2 Follow safety guidelines in the community, including proper lifting technique and universal precautions when providing care to residents. 3 Participate as a team, assisting coworker as the need arises to ensure that the needs of all residents living in the community are met. 4 Monitor resident activity, functional status, psychological status, and assisting as required or needed to promote resident health and well being. 5 Report any changes of condition and/or falls immediately to Supervising LPN/ Med Tech. 6 Respond immediately, while following community protocol, to any resident crisis and then report to Supervising LPN/ Med Tech including any first aid services administered from basic first aid training. 7 Document any resident status change on the Daily Communication Log for the next shift. 8 In the event all assigned duties cannot be completed, ask for assistance, write on Daily Communication Log, report to the next shift and Supervising LPN/ Med Tech. 9 Complete any other assignments as determined by the Director of Nursing, Supervising LPN/ Med Tech, or any other Director/Program Manager within your scope of practice. 10 Promote open communication between health care professionals, families, residents, and staff. 11 Adhere to ALL guidelines in the employee handbook including dress code, conduct, scheduling, confidentiality, HIPPA, etc. 12 Other duties as assigned within your scope of practice. (i.e. plunging toilets, laundry, mopping floors, dishes, serving and dining services, etc.)Physical Requirements Ability to lift, push or pull up to 50 lbs., using proper body mechanics and/or available equipment Ability to kneel for 30+ consecutive minutes to perform ADL's and other tasks assigned or needed Ability to work standing, walking or moving for up to 12 hours or entire shift(s) without need to sit down
