Job Summary:
The Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their place of residence in accordance with the patient's plan of care. The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety. The CNA plays an important role in supporting hospice patients and families by assisting with daily care needs and reporting changes in patient condition to the clinical team. This role requires strong patient care skills, compassion, reliability, and the ability to work as part of the hospice care team.
Qualifications:
Current state certification as a Certified Nursing Assistant (CNA) required Hospice, home health, long-term care, or healthcare experience preferred Strong communication and patient care skills Ability to follow patient care plans and instructions from nursing staff Ability to work independently in a home or facility setting Reliable transportation, valid driver's license, and current auto liability insurance Ability to read, write, and communicate effectively in
English Essential Functions:
Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance Assist patients with activities of daily living (ADLs) according to the plan of care Document patient care and observations accurately and timely Report changes in patient condition or environment to supervising nurse Promote patient safety, comfort, dignity, and independence Maintain a clean and safe patient environment Follow infection control and safety procedures in all patient care activities Assist with safe transfers and mobility using proper lifting techniques Attend in-service training and maintain CNA certification requirements Communicate effectively with patients, families, nurses, and interdisciplinary team members Follow company policies, procedures, and patient care standards Perform other related duties as assigned Excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.
