Humble, Texas Position Summary Elite Care HCS Inc. is seeking a compassionate, dependable, and experienced Certified Nursing Assistant to provide direct residential support to an individual with intellectual and developmental disabilities in a group-home setting. The home currently serves one individual. The CNA will assist with all activities of daily living, health and safety needs, household routines, community participation, and other supports identified in the individual's service plan. Responsibilities may expand as additional individuals are admitted to the home. Essential Duties and Responsibilities Assist with all activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and personal care. Provide supervision and support throughout the assigned shift. Assist with mobility, transfers, positioning, and ambulation as needed. Prepare meals and provide feeding assistance according to the individual's dietary needs. Complete laundry, light housekeeping, sanitizing, and other household responsibilities. Support the individual with maintaining a clean, safe, and comfortable living environment. Assist with medications only after completing all required training and receiving proper authorization or nursing delegation. Monitor and document changes in the individual's health, behavior, appetite, mobility, mood, and overall condition. Immediately report medical concerns, injuries, incidents, medication issues, behavioral concerns, or unusual changes to the appropriate supervisor or nurse. Follow the individual's service plan, behavior-support plan, dietary requirements, safety plan, and established daily routine. Transport or accompany the individual to medical appointments, community activities, errands, and outings when assigned. Encourage independence, personal choice, social interaction, and participation in meaningful activities. Complete accurate service-delivery notes, shift documentation, medication records, incident reports, and other required records before the end of each shift. Maintain confidentiality and protect the individual's privacy and personal information. Follow all company policies, infection-control procedures, emergency protocols, and abuse, neglect, and exploitation reporting requirements. Remain awake, alert, and attentive throughout the entire assigned shift. Perform other duties related to the individual's care and operation of the group home as assigned. Minimum Qualifications Current Certified Nursing Assistant certification in good standing. High school diploma or GED. At least one year of CNA, direct-care, home-health, residential, or caregiving experience preferred. Experience supporting individuals with intellectual and developmental disabilities is strongly preferred. Current CPR and First Aid certification, or willingness to obtain certification before beginning independent shifts. Ability to work 8- to 12-hour shifts, including some weekends and holidays. Ability to communicate professionally with individuals, guardians, family members, nurses, supervisors, and other members of the care team. Ability to follow written service plans, medical instructions, and company procedures. Ability to complete accurate written or electronic documentation. Valid driver's license, current automobile insurance, and reliable transportation. Ability to successfully complete all required background checks, registry checks, and pre-employment screenings. Physical Requirements Ability to stand, walk, bend, reach, and remain active for extended periods. Ability to assist with personal care, mobility, positioning, and transfers. Ability to lift or support the weight required to safely meet the individual's needs, with or without reasonable accommodation. Ability to respond promptly and appropriately during medical, behavioral, or environmental emergencies.
