Duties Assist patients with daily activities such as bathing, dressing, feeding, and mobility to promote independence and dignity Monitor vital signs including blood pressure, pulse, respiration, and temperature; document findings accurately in electronic health records (EHR systems) Support medication administration under the supervision of nursing staff, ensuring proper techniques and adherence to HIPAA regulations for patient confidentiality Utilize safe lifting practices and equipment like Hoyer lifts to transfer patients safely and prevent injury Observe and report changes in patient conditions promptly to the nursing team, including signs of distress or deterioration Maintain a clean, safe environment by following aseptic techniques and infection control protocols; assist with wound care and catheter care as needed Collaborate effectively with multidisciplinary teams to coordinate patient care plans in medical-surgical, long-term care, or assisted living settings Skills Proven experience working with geriatric patients and familiarity with dementia, Alzheimer's, memory care, or behavioral health needs Knowledge of EMR/EHR systems such as Epic or Cerner for accurate medical documentation and record management Strong understanding of vital signs measurement, anatomy knowledge, and physiology fundamentals relevant to patient care Ability to perform heavy lifting safely and handle manual transfers using appropriate equipment like Hoyer lifts Excellent communication skills for caregiver-patient interactions and interdisciplinary team collaboration Basic life support (BLS) certification required; CPR certification preferred Experience working in diverse healthcare environments including hospital medicine, ICU, emergency medicine, or post-acute care settings is highly valued Join us in delivering compassionate care that transforms lives!
