Responsibilities Deliver personalized patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts and other assistive devices. Monitor vital signs including blood pressure, pulse, temperature, and respiration; accurately document findings using Electronic Health Record (EHR) systems like Point Click Care (PCC). Support patients with specialized needs such as dementia care, Alzheimer's care, memory care, or behavioral health challenges by implementing tailored interventions. Operate medical equipment such as catheters, and monitors; ensure proper use and safety standards are upheld at all times. Collaborate effectively with multidisciplinary teams to ensure comprehensive patient service across inpatient, outpatient, home care, or assisted living environments. Qualifications Valid CNA certification with current state licensure; additional certifications such as Basic Life Support (BLS) preferred. Familiarity with EMR/EHR systems such as Point Click Care (PCC) for accurate medical documentation. Knowledge of physiology, anatomy, medical terminology, and aseptic techniques essential for safe patient care delivery. Experience supporting individuals with disabilities or developmental challenges; including dementia and Alzheimer's disease care is highly valued. Ability to perform vital signs measurement and basic patient assessments accurately; proficiency in infection control practices is required. Strong caregiving skills complemented by excellent communication abilities to foster trust and comfort with patients of all ages including pediatrics and geriatrics.
