Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using safe lifting practices and equipment like Hoyer lifts. Monitor and record changes in patient condition accurately within electronic health record (EHR) systems. Provide emotional support and compassionate communication to patients and their families during difficult times, fostering a comforting environment. Collaborate with nurses to implement personalized care plans tailored to each patient's needs. Maintain patient confidentiality by adhering strictly to HIPAA guidelines and safeguarding medical records across all documentation platforms such as EMR/EHR systems. Perform specialized care tasks including catheter care, infection control using aseptic techniques, wound management, and dementia or Alzheimer's care strategies. Assist with transfers using patient transfer methods appropriate for each individual's mobility level while practicing safe manual handling techniques. Participate in ongoing training related to emergency medicine protocols such as CPR, basic life support (BLS), and infectious disease care to ensure readiness for urgent situations. Qualifications Current certification as a Nursing Assistant (CNA) with valid state licensure required. Demonstrated experience working with geriatric patients in hospice, long-term care facilities, or home care environments preferred. Familiarity with EMR/EHR systems such as Consolo for accurate medical documentation. Experience working with individuals with disabilities or behavioral health needs is highly desirable. Knowledge of medical terminology, anatomy, physiology, and patient transfer methods essential for delivering safe care. Prior experience in diverse healthcare settings including hospital medicine, ICU, emergency medicine, or post-acute care is advantageous. Strong communication skills to effectively coordinate with team members and provide caregiver support. Ability to perform heavy lifting safely while maintaining proper body mechanics during patient transfers or manual handling tasks. Commitment to maintaining patient confidentiality under HIPAA standards and adhering to infection control protocols such as aseptic technique.
