Job Summary Join our compassionate healthcare team as a Home Health Aide, Caregiver, and CNA, where you will play a vital role in delivering personalized, high-quality care to individuals in their homes and community settings. This dynamic position offers an opportunity to make a meaningful difference in patients' lives by providing essential support with daily activities, health monitoring, and emotional well-being. You will work closely with clients of all ages, including seniors, individuals with disabilities, and those receiving hospice or palliative care, ensuring they maintain independence and dignity in a safe environment. This paid position emphasizes professional growth, teamwork, and the rewarding experience of caregiving. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by taking vital signs like blood pressure, pulse, respiration rate, and temperature; document medical records accurately using Electronic Health Record (EHR) systems while maintaining patient confidentiality in compliance with HIPAA regulations. Administer medications following prescribed protocols and assist with medication management to ensure safety and adherence. Provide specialized care such as Alzheimer's care, dementia care, hospice support, end-of-life assistance, catheter care, and memory care tailored to individual needs. Support clients with household tasks including meal preparation, cooking, cleaning, laundry, and maintaining a safe living environment. Observe patients closely for changes in condition or behavior; communicate effectively with clients and family members to ensure clarity about care plans. Follow home healthcare regulations and safety procedures during patient transfers or when operating medical equipment like Hoyer lifts or other assistive devices. Respond promptly to emergencies by performing first aid or basic life support (CPR), ensuring the safety of clients at all times. Experience Prior experience working with geriatric patients or in assisted living facilities is highly preferred. Demonstrated knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes. Experience assisting individuals with disabilities or developmental challenges enhances your ability to provide comprehensive care. Background in long-term care settings such as nursing homes or home healthcare agencies is advantageous. Proven ability to perform patient personal care assistance with compassion and professionalism while maintaining strict confidentiality under HIPAA guidelines. Hands-on experience with hospice or palliative care services is a plus. Valid certifications in first aid and CPR are required; additional certifications such as CNA licensure or Home Health Aide training are highly desirable.
