As an AIDE of Kindful Health you will work with the RN Case Manager to provide best in class care to a reasonably sized case load. The AIDE will present at IDT every 2 weeks with updates from their fellow care team members and coordinate with the RN Case Manager, Clinical Director and Medical Director to assure best in class care.Essential Education and Experience :
Must be a certified as a nurse aide with a current license and current with your continuing educationResponsibilities & Duties:
The AIDE will be responsible for: Adheres to all regulatory requirements and professional standards Follows policies and procedures consistent with recognized standards of professional practice Identifies system problems or inefficiencies that may negatively affect patient care Manage clinical excellence within your patient census. Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores Collaborate with your RN Case Manager, Clinical Director, Medical Director and entire team to provide a "concierge" level hospice experienceQualifications & Skills :

Hourly- $22•$23 per hour.
