Role:
Assist residents with daily living activities, such as bathing, dressing, and grooming, in accordance with their individual needs and care plans. Provide emotional support and companionship to residents, engaging in conversations and activities that promote mental and emotional well-being. Assist with meal preparation, feeding, and feeding assistance as needed. Observe and report any changes in resident condition, behavior, or health to licensed nursing staff. Support the delivery of person-centered care by actively listening to and responding to residents' needs and concerns.
Responsibilities:
Maintain accurate and timely documentation of resident care, including daily logs and incident reports. Participate in care conferences and multidisciplinary team meetings to discuss resident care and concerns. Effectively communicate with residents, families, and other healthcare professionals to ensure seamless care coordination. Maintain a safe and clean environment, adhering to infection control and safety protocols.
