In this vital role, you will provide exceptional patient care, assist with daily living activities, and support individuals in various care settings. Duties Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, reporting changes, and maintaining detailed medical documentation within electronic health record (EHR) systems. Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and end-of-life support. Support patients with mobility and transfer techniques while ensuring safety protocols are followed to prevent falls or injuries. Maintain patient confidentiality by adhering to HIPAA guidelines and safeguarding sensitive medical records. Assist with meal preparation, light cleaning, laundry, and overall home & community care tasks to promote a safe and comfortable environment. Skills Strong patient observation skills to detect changes in condition or behavior promptly. Experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Knowledge of medical terminology, vital signs measurement, medication administration, and first aid/CPR certification. Ability to perform heavy lifting safely using proper techniques and equipment such as Hoyer lifts when necessary. Excellent communication skills for client interaction, family updates, and interdisciplinary team collaboration. Understanding of home health care regulations, HIPAA compliance, and best practices in senior care and dementia management. Join us in delivering compassionate care that truly makes a difference!
