Strong verbal and written communication skills Dependable, responsible, and self-motivated New graduates are welcome — training will be provided Compassionate and empathetic approach to patient care Strong attention to detail and ability to meet deadlines Maintains a high level of integrity and ethical standards Works effectively both independently and as part of a team Basic computer and technology skills Reliable transportation CNA Responsibilities Provide CNA-level care to home health and hospice patients in their homes or other places of residence, within the scope of training and certification. Perform covered aide services according to Medicare home health and hospice guidelines. Assist patients with activities of daily living and other personal care needs based on assessments, individualized care plans, and direction from nursing management. Provide care under the supervision and direction of a Registered Nurse. Develop positive and professional relationships with patients, families, physicians, interdisciplinary team members, and other healthcare professionals. Monitor patients and promptly report changes or concerns to the appropriate member of the care team. Accurately document observations, services provided, and patient care in a timely manner. Safely and appropriately use patient-care equipment within the CNA's training and competency, including wheelchairs, hospital beds, side rails, oxygen equipment, and other applicable medical equipment. Follow established care plans, safety procedures, and organizational standards while providing respectful and compassionate care. Required Licenses & Certifications Active Utah CNA certification Current CPR certification Valid Utah driver's license Current automobile insuranceJob Type:
