This vital role involves providing personalized patient care, assisting with daily activities, and ensuring comfort and safety for individuals across various care settings. As a caregiver, you'll help older adults stay safe, comfortable, and connected at home. Depending on each client's needs, your work may include companionship, meal preparation, help with daily routines and personal care, reminders, and outings. Whether working in home health, assisted living, or long-term care facilities, you'll be a trusted partner in promoting independence and well-being. Responsibilities Deliver direct patient care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, managing catheter care, and reporting changes promptly Assist with medication reminders following medical documentation protocols and home healthcare regulations Support patients with memory care needs, dementia, Alzheimer's, and end-of-life care to ensure comfort and dignity Utilize EMR (Electronic Medical Record) systems to accurately document patient information and medical records Implement patient transfer methods safely using equipment like and patient transfer techniques Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients and their families Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients Adhere to all safety standards including first aid, CPR certification, and basic life support procedures to handle emergencies confidently Support clients' emotional well-being through compassionate communication and personalized care approaches. You may be a great fit if you: Treat clients with patience, warmth, and respect Arrive reliably for the shifts you accept Communicate promptly with the office about client needs or schedule concerns Have professional or personal caregiving experience Experience Prior experience working with geriatric patients in settings such as nursing homes or assisted living communities is highly preferred Demonstrated experience assisting individuals with disabilities or developmental challenges is advantageous Knowledge of medical terminology relevant to home health care and hospice environments is beneficial Proven ability to work independently or as part of a team in home health agency or long-term care settings Experience operating EMR/EHR systems for accurate medical documentation and record keeping Familiarity with home & community care standards, hospice care practices.
