In this role, you'll provide one-on-one support that helps clients remain safe, comfortable, and independent in their homes or residential care settings. Key Responsibilities Under the direction of a Nurse Manager and according to each client's personalized care plan, you will: Deliver compassionate companionship and daily living assistance. Assist with mobility, including transfers, ambulation, repositioning, and wheelchair support. Help plan meals, shop for groceries, and prepare nutritious food. Provide personal care such as bathing, dressing, grooming, toileting, and oral hygiene. Complete light housekeeping duties, including laundry and maintaining a clean living environment. Remind clients to take prescribed medications and assist with self-administration when appropriate. Observe and document changes in client condition, including intake and output as directed. Escort clients to medical appointments or assist with essential errands when needed. Documentation & Team Communication Accurately document services using the company's electronic visit verification (EVV) or timekeeping system. Immediately communicate any concerns regarding a client's health, safety, or well-being to the supervising nurse or management. Attend required staff meetings and complete annual in-service education, including a minimum of 12 continuing education hours. Minimum Requirements Must be at least 18 years of age. High school diploma, GED, or equivalent. Current PCA certification or active CNA license. At least one year of professional caregiving experience preferred. Valid driver's license with reliable transportation. Current automobile insurance.
