Schedule:

Various shifts may be available, including days, evenings, weekends, and holidays What You'll Do Provide direct care, support, and assistance to individuals in a community-based setting Assist with daily living activities and personal care as needed Encourage independence, dignity, and participation in community activities Provide transportation or accompany individuals to appointments and activities when required Monitor and document individual activities and services provided Follow individualized service plans and established safety procedures Communicate effectively with supervisors, coworkers, individuals served, and their families/support systems Maintain a safe, respectful, and supportive environment What We're Looking For Compassionate and patient approach to supporting others Dependable attendance and strong work ethic Ability to communicate effectively and work as part of a team Ability to maintain confidentiality and professionalism Previous experience in healthcare, behavioral health, human services, or direct care is a plus High school diploma/GED may be required depending on assignment Must be able to successfully complete required background checks and pre-employment requirements
