Hospice Certified Nursing Assistant (CNA)Lake Prince at Home | Suffolk, VAFull-Time PositionAt Lake Prince at Home, we believe hospice care is about providing dignity, comfort, and compassionate support during life's most important moments. We're seeking a dedicated Hospice Certified Nursing Assistant (CNA) to join our caring team and make a meaningful difference in the lives of patients and their families.

Position SummaryThe Hospice CNA provides personal care services to hospice patients under the supervision of a Registered Nurse and in accordance with physician orders and individualized care plans. This role focuses on enhancing patient comfort, promoting dignity, and supporting families during end-of-life care.
Essential ResponsibilitiesProvide personal care assistance, including bathing, dressing, grooming, toileting, and mobility support.
Observe and report changes in patient condition, physical status, and emotional well-being to the hospice care team.
Document patient care activities and relevant observations accurately and timely.
Follow established hospice care plans and nursing instructions.
Assist patients in maintaining comfort and quality of life.
Support family members and caregivers through compassionate communication and care.
Maintain regular communication with the RN, Clinical Coordinator, and interdisciplinary hospice team.
Participate in staff meetings, training programs, and continuing education opportunities.
Complete scheduled patient visits in accordance with agency standards and patient needs.
QualificationsCurrent Virginia Certified Nursing Assistant (CNA) certification required.
Valid driver's license, reliable transportation, and auto insurance.
Ability to travel to patient homes throughout the service area.
Strong communication and interpersonal skills.
Compassionate approach to caring for patients and families facing end-of-life challenges.
Hospice, home health, or long-term care experience preferred.
