Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices. Monitor vital signs, record medical documentation accurately, and report changes in patient conditions promptly. Support medication administration under supervision, ensuring adherence to HIPAA regulations and proper medical-surgical procedures. Provide specialized care for individuals with dementia, Alzheimer's disease, memory impairments, or behavioral health needs. Maintain aseptic techniques during procedures like catheter care and phlebotomy to prevent infection and ensure patient safety. Utilize electronic health record (EHR) systems such as Epic or Cerner for documentation and communication across medical teams. Collaborate with multidisciplinary teams in acute care, ICU, emergency medicine, pediatrics, geriatrics, hospice, and behavioral health settings to deliver comprehensive patient service. Qualifications Valid certification as a Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA), with active state licensing where applicable. Experience working in diverse healthcare environments including hospitals (Level I Trauma Center or Level II Trauma Center), nursing homes, assisted living facilities, or home care settings. Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for medical documentation and records management. Knowledge of medical terminology, anatomy, physiology, and infection control practices including aseptic technique. Ability to perform vital signs measurement and basic life support (BLS) certification is preferred. Experience caring for individuals with developmental disabilities or behavioral health conditions is a plus.
