Job Description
ADL care, providing residents with necessary assistance and required care services Ensure residents receive assistance with care, to whatever level required and/or ordered. Resident tasks are completed and documented within EMR with accuracy. Vital signs and weights are obtained and documented within EMR. Behaviors are monitored, documented and the charge nurse notified as needed. Monitor resident safety Resident safety always a priority, from monitoring and preventing falls, ensuring all safety devices are in place and functioning properly, special alarms and call bells are in place, bed positioning is correct and transfers are completed as required. Use of lifts are completed with proper equipment and staff assistance. Make regular rounds Rounds are made at least every two hours to monitor residents assigned. Call lights are answered promptly, with courtesy and care provided as needed. Documentation completed as needed. Encourage residents to participate in activities and therapies, plus socialization in common areas. Observe and report resident changes, symptoms, incidents or concerns to the charge nurse Monitor and observe resident's physical, mental and emotional conditions, reporting changes immediately to the charge nurse. Documentation completed regarding observations to ensure compliance and trigger of alerts. Assist with resident toileting, as necessary Compliance with facility policy regarding toileting and incontinence. Follow bowel and bladder protocols for residents assigned. Accurately monitor for bowel issues, reporting concerns to charge nurse. Monitor for lack of urine output. Accurately maintain Intake & Output. Ensure proper foley catheter placement and monitoring. Assist with proper nutritional needs for residents Monitor for proper diet, or if diet issues are noted. Assist with feeding residents as needed. Ensure proper hydration, with fresh water available and within reach. Accurately maintain Intake and Output. Monitor skin and alert charge nurse to concerns. Turn residents as ordered to prevent skin breakdown. Assignments- Complete assignments as ordered, performing cleaning, stocking, and keeping resident areas orderly and neat. Participate in Emergency Preparedness drills as required. May be assigned to any unit as staffing needs change with short notice. Essential10 Other Duties May be required to assist the agency or state government generally in the event of an emergency declaration by the Governor.
