In this vital role, you will provide compassionate, personalized care to individuals in various settings, including assisted living, nursing homes, home healthcare, and hospice. Your efforts will help enhance quality of life, promote independence, and ensure comfort for patients with diverse needs. This position offers a rewarding opportunity to support community members through attentive caregiving and professional dedication. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature. Support medication administration following medical documentation protocols while maintaining patient confidentiality in accordance with HIPAA regulations. Observe patient conditions closely, report changes promptly, and document interactions. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies. Assist with catheter care, wound dressing changes, and end-of-life comfort measures in hospice or palliative settings. Maintain a clean and safe environment by assisting with laundry, cleaning duties, meal preparation, and ensuring adherence to home health care regulations. Requirements Valid certification as a Certified Nursing Assistant (CNA), Direct Care Aide (DCA), or Personal Care Assistant (PCA). Willing to train the right individuals. Prior experience working in assisted living facilities, nursing homes, or home healthcare environments preferred. Knowledge of medical terminology and procedures related to patient care. Experience working with Adults with developmental disabilities or geriatrics is highly desirable. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Familiarity with home health agency operations and home & community care standards. Strong communication skills for effective client interaction and team collaboration.
