Monday thru Friday 8:45-5:15 To provide health related services under the direction of staff RN's as coordinated by the Program Supervisor and facilitate activities in cooperation with the staff members responsible for the program's Activity Program appropriate to all participants' needs in accordance with goals in the Plans-of-Service. To assist all participants, including those with dementia, as needed, as to Activities of Daily Living (ADL) and activities that are designed to promote personal growth and improve or maintain the functioning level of participants to the extent possible. State and Federal Guidelines Duties & Responsibilities :

Assist participants with: Toileting & personal care needs (i.e. shampooing, fingernails, bathing, etc.) Ambulation, transfers, adaptive equipment Meal preparation and set up including assist with feeding as needed Exercise program - both group and individually Group or individual activities Maintain supervision of and redirection of participants at-risk for elopement Participate in team coordination/development of Plans-of-Service Assist with site/facility clean-up and safety maintenance and compliance (Immediately report any areas or issues not in compliance), inventory maintenance. Report any changes in participants' conditions and/or unusual behavior to RN/Nursing Staff and Program Supervisor. Assist with program development and community outreach, including fund-raising activities. Knowledge of client/participant charting procedures. Chart in-put as appropriate Attend and participate in staff meetings and in-service trainings Interaction with other staff members, caregivers and assist in coordination/supervision/utilization of volunteer services and program assistants Perform other related tasks as requested or necessitated under the direction of the supervisor(s)
