Responsibilities & Duties Observe and report any changes in patients' conditions to the nursing staff. Assist resident with dressing, grooming, oral hygiene, bathing, hair, incontinence care etc. Provide support to residents with behavioral health needs. Take and record temperature, blood pressure, pulse, respiration rate, fluid intake and output as directed. Assist residents with range of motion exercises, eating, and mobility/ transfers.
