In this vital role, you will provide personalized patient care, assist with daily living activities, and support overall well-being within various care settings such as assisted living, nursing homes, or home health environments. Your energetic approach and attention to detail will ensure residents receive respectful, high-quality care while promoting their independence and comfort. Duties Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' vital signs, observe for changes in condition, and report findings accurately to nursing staff. Administer medications and assist with medication management in accordance with care plans and HIPAA regulations to ensure confidentiality and safety. Support residents with specialized needs such as dementia, Alzheimer's disease, or neurodevelopmental disorders through tailored memory care and dementia care techniques. Prepare nutritious meals aligned with dietary restrictions and assist with meal service to promote health and well-being. Maintain a clean, safe environment by performing light cleaning duties, sanitation of patient areas, and ensuring compliance with health standards. Document patient observations, care provided, and any incidents thoroughly to support ongoing care plans and resident rights. Provide transportation assistance for appointments or outings, ensuring safety during transfers and travel. Engage in patient monitoring to identify potential issues early while fostering positive communication with clients to support their emotional needs. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated experience providing patient personal care assistance, including helping with ADLs and mobility transfers. Knowledge of HIPAA regulations related to patient privacy and confidentiality is essential. Experience working with individuals with autism, intellectual disabilities, developmental disabilities, or neurodevelopmental disorders is advantageous. Familiarity with medication administration, vital signs measurement (such as blood pressure or pulse), CPR/First Aid certification is required or highly desirable. Background in geriatrics or hospice care enhances your ability to deliver compassionate senior care. Ability to perform heavy lifting safely when assisting residents during transfers or repositioning is necessary.
