Shift Differentials:

Rocky Knoll is hiring Certified Nursing Assistants with all of the above. The Certified Nursing Assistant (CNA) provides direct, hands-on care to residents under the supervision of licensed nursing staff to support comfort, dignity, and quality of life. This position assists residents with activities of daily living, observes and reports changes in condition, and contributes to individualized care plans in accordance with facility policies and regulatory standards. CNAs play a vital role in maintaining a safe, clean, and supportive environment while promoting resident independence and wellbeing. Work is performed collaboratively within an interdisciplinary team focused on delivering compassionate, person-centered care to all residents of the Health Care Center.
  • Provide direct, person-centered care to residents under the supervision of licensed nursing staff in accordance with individualized care plans, state and federal regulations, and facility policies.
  • Assist residents with daily living activities including bathing, dressing, grooming, toileting, oral hygiene, mobility, and feeding while promoting independence and dignity.
  • Observe, monitor, and report changes in residents' physical, emotional, or behavioral condition to the charge nurse in a timely manner.
  • Complete required documentation and charting accurately and promptly, including meal intake, activities of daily living (ADLs), and behavior or condition changes.
  • Provide assistance with restorative and rehabilitative programs such as range-of-motion exercises, ambulation, and positioning.
  • Ensure resident comfort by repositioning, adjusting bedding and equipment, and maintaining a safe and sanitary living environment.
  • Assist with transporting residents to and from meals, activities, and therapies.
  • Participate in resident mealtime routines by preparing trays, serving meals, feeding residents as needed, and observing dietary restrictions and swallowing precautions.
  • Monitor safety during meals to prevent choking and follow diet orders and physician instructions.
  • Perform routine housekeeping tasks on the unit such as making beds, changing linens, cleaning wheelchairs and equipment, and maintaining neat and organized resident areas.
  • Respond promptly to call lights, alarms, and resident requests, ensuring needs are met efficiently and respectfully.
  • Record and report vital signs, weights, intake/output, and other relevant resident data per facility protocol.
  • Support infection prevention practices and follow all safety, sanitation, and universal precaution guidelines, including use of Personal Protective Equipment and enhanced barrier precautions .
  • Assist nursing staff in emergency situations, including falls, medical incidents, and resuscitation measures as trained and directed.
  • Participate in staff meetings, care conferences as needed, and in-service education sessions to maintain required certifications.
  • Maintain supplies and equipment; report maintenance or safety concerns promptly to supervisory staff.
  • Promote resident rights, privacy, and confidentiality at all times.
  • Perform other related duties as assigned to ensure the continuity and quality of resident care.
Attending high school or a high school diploma or equivalent. One (1) year of experience in a healthcare setting. A combination of education and experience may be considered. Must possess current certification as a Certified Nursing Assistant (CNA) in good standing with the State of Wisconsin Department of Health Services. CPR and First Aid certification may be required after hire in accordance with facility policy.
  • Work is performed primarily in a skilled nursing and rehabilitation facility with exposure to residents, staff, medical equipment, cleaning agents, and bodily fluids.
  • Duties require frequent standing, walking, bending, stooping, kneeling, pushing, pulling, and reaching throughout the workday.
  • Must be able to lift, move, and reposition residents or equipment weighing up to 50 pounds independently and more with assistance or mechanical lift devices.
  • Regularly assists in resident transfers using proper body mechanics and assistive equipment to prevent injury.
  • Requires visual and auditory acuity sufficient to monitor resident safety, recognize changes in condition, and accurately read and document information.
  • Work involves potential exposure to infectious diseases, bloodborne pathogens, cleaning chemicals, and other occupational health hazards; appropriate personal protective equipment (PPE) must be used as required.
  • May be exposed to varying temperatures and noise levels in resident rooms, dining areas, and common spaces.
  • Requires manual dexterity for operating medical and mobility equipment, documenting in electronic health record systems, and performing resident care tasks.
  • Work schedule may include days, evenings, nights, weekends, and holidays based on facility needs.
  • Risk of bloodborne pathogen exposure is considered high.
