Behavioral Health Care Manager (Collaborative Care Model - CoCM) Helena, Arkansas Full-Time | Exempt East Arkansas Family Health Center is seeking a compassionate and organized Behavioral Health Care Manager to join our integrated Behavioral Health team. This position plays a key role in our Collaborative Care Model (CoCM) by coordinating behavioral and physical healthcare services for patients experiencing mental health and substance use disorders. Screen and assess patients for common mental health and substance use disorders. Educate patients and families regarding behavioral health conditions and available treatment options. Monitor patient progress, treatment response, and medication adherence through in-person and telephone follow-up. Provide brief evidence-based behavioral interventions, including Motivational Interviewing, Behavioral Activation, and Problem-Solving Therapy. Collaborate with primary care providers and psychiatric consultants to develop and implement individualized treatment plans. Participate in weekly Collaborative Care case review meetings with the psychiatric consultant. Maintain patient registries and track clinical outcomes to ensure timely follow-up and quality care. Coordinate referrals to specialty behavioral health services, substance use treatment programs, community resources, and social service agencies. Assist eligible patients with enrollment in East Arkansas Family Health Center's Patient Assistance Program. Connect patients with community resources including housing assistance, transportation, financial assistance, vocational rehabilitation, and other supportive services. Develop relapse prevention and self-management plans for patients successfully completing treatment. Maintain accurate documentation in the Electronic Health Record (EHR) and other required systems. Participate in community outreach activities, quality improvement initiatives, and program development. Prepare monthly reports and other program documentation as required. Maintain compliance with HIPAA, OSHA, and all applicable federal and state regulations. Perform other duties as assigned. Qualifications Required Current Arkansas licensure in one of the following: Licensed Master Social Worker (LMSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT) Licensed Social Worker (LSW) Registered Nurse (RN) (BSN preferred) Nurse Practitioner (NP) Licensed Psychologist Experience in behavioral health, care management, case management, counseling, nursing, or a related healthcare field. Knowledge of behavioral health assessment, treatment planning, and care coordination. Experience with evidence-based behavioral interventions such as Motivational Interviewing, Behavioral Activation, or Problem-Solving Therapy. Strong communication, organizational, and interpersonal skills. Ability to collaborate effectively within an interdisciplinary healthcare team. Preferred Experience working in a Federally Qualified Health Center (FQHC) or community health center. Experience working within a Collaborative Care Model (CoCM). Experience with Electronic Health Record (EHR) systems. Experience coordinating behavioral health and primary care services. Bilingual skills are a plus.
