We are committed to caring for all our clients as if they were part of our own family. We are seeking a Care Manager to join our team in Silicon Valley. This role is ideal for someone passionate about delivering exceptional client experience.
Key Responsibilities Care Management:
Oversee the ongoing management of client services, ensuring their needs and expectations are consistently met. Act as the main point of contact for clients and their families, addressing concerns and resolving issues with empathy and efficiency. Conduct assessments and reassessments for new clients and assist in developing personalized care plans. Maintain detailed records and communication with caregivers and the internal team to deliver seamless care. Recommend and support the implementation of caregiver training initiatives, including specialized Alzheimer's/Dementia care programs. Who You Are We're seeking a compassionate, goal-driven professional delivering exceptional client care.
Requirements:
Bachelor's degree, RN or LVN (preferred), or equivalent experience in healthcare/home care. Preferred 2 years of experience in care management, ideally in healthcare, home care, or hospice. Strong interpersonal and communication skills with the ability to connect with diverse audiences. Detail-oriented, adaptable, and able to thrive in a fast-paced environment. Valid driver's license and willingness to travel within Santa Clara and San Mateo Counties.

Pay Range:
