$22 per hour About Optima Health Optima Health provides mobile primary care services to senior living communities and patients across Southern California. Our team brings medical care directly to residents, helping improve access, continuity, and support for older adults in senior living settings. We are seeking a reliable, compassionate, and organized Care Manager to support our providers throughout Riverside and Moreno Valley. This role is ideal for someone who enjoys working with seniors, supporting clinical care, and contributing to a mobile healthcare team. Position Overview The Care Manager plays an essential role in supporting providers who deliver medical care directly in the community. This field-based position involves traveling to senior living communities to assist with resident care, clinical coordination, documentation support, and the provider's daily workflow. The ideal candidate is professional, dependable, detail-oriented, and comfortable working in a mobile environment. This role requires strong communication skills, compassion for older adults, and the ability to support a smooth and positive patient care experience. Key Responsibilities Travel with or alongside providers to senior living communities throughout Riverside and Moreno Valley Support providers during patient visits and daily clinical operations Assist with resident intake, basic patient information, and visit preparation Gather patient histories, medication information, and relevant care updates Take and record vital signs as needed Support documentation, chart preparation, and follow-up tracking Communicate professionally with residents, families, facility staff, and internal team members Help coordinate referrals, labs, diagnostics, and follow-up items as directed Maintain organized records of patient visits and provider support needs Assist with scheduling, visit flow, and patient communication when needed Identify and report changes in patient condition or other concerns to the provider Help ensure a smooth, respectful, and compassionate care experience for residents Maintain patient confidentiality and follow HIPAA requirements Represent Optima Health professionally within senior living communities Qualifications Experience in senior care, primary care, geriatrics, home health, assisted living, or mobile healthcare preferred Strong communication and interpersonal skills Compassionate and patient-centered approach to working with older adults Highly organized, dependable, and detail-oriented Comfortable working independently and supporting providers in the field Ability to manage multiple tasks and follow through on details Professional appearance and demeanor Basic computer and documentation skills Reliable transportation required Ability to travel regularly throughout Riverside and Moreno Valley Compensation and Benefits $22 per hour Health, dental, and vision insurance Paid time off Retirement plan Professional development assistance Mileage expense reimbursement Why Join Optima Health Support meaningful care for seniors in the community Work directly with providers and senior living teams Help improve access to primary care for older adults Join a growing mobile healthcare organization Build valuable experience in clinical operations, care coordination, and senior care Work Location This is a field-based role serving Riverside and Moreno Valley.
