Hours:

40 hours per week; Monday - Friday, 8:00 am to 4:30 pm As a Lead Care Manager I, you will: Provide mobile outreach and engagementScreen and assess for program eligibilityPromote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionalsConnect clients and their families to a medical clinic and providers, as well as other clinic and community-based servicesArrange transportation and accompany clients to office visits as neededServe as an advocate for the client and familyUse motivational interviewing, trauma-informed care, and harm-reduction practicesCreate and maintain all required documents in the electronic and paper record systems
