Institute on Aging is looking for a Care Manager to join our team, supporting older adults and adults with disabilities so they can live safely and independently in their own homes. What You'll Do Conduct comprehensive intake assessments and develop client-centered Community Living Plans Connect clients to community resources and entitlement programs such as IHSS, Medicare, and Medi-Cal Build trusted, respectful relationships with clients and their families Provide crisis intervention, advocacy, and problem-solving support as needed Document all case management activity and maintain clear, consistent records Coordinate with physicians, community providers, and other health and social service professionals Work independently while staying connected to a small, collaborative team What You Bring BA or BS in Social Work or a related field 1 year of experience working with disabled adults and/or the frail elderly Understanding of the medical and psychosocial needs of functionally impaired adults and older adults Strong time management skills and comfort juggling multiple priorities Familiarity with SF community resources, housing, and mental health services a plus Bilingual candidates highly preferred: English plus Cantonese, Mandarin, and/or Spanish Comfort with fieldwork and home visits, plus a valid ability to travel around San Francisco Computer literacy and the ability to lift up to 25 lbs, reach, bend, and walk Why IOA Join a small, tight-knit team that shows up for each other as much as they show up for clients.
