JOB SUMMARY

The Residential Care Manager works with individuals in OUD, STUD, and other SUD recovery and behavioral health recovery. The Care Manager provides recovery-oriented services, integrating evidenced-based practices which are effective with a chronically homeless population with severe mental illness. Such evidence-based practices include, but are not limited to Recovery Support services, Motivational Interviewing, Recovery-Oriented Mental Health Services, Crisis Intervention, and Trauma-Informed Care.

ESSENTIAL FUNCTIONS

1. Perform initial comprehensive assessments 2. Develop a plan of intervention that addresses identified need for housing, on-going care management services, mental health services, and coordination of needed community services 3. Develop individualized care plan in collaboration with client addressing short term and long term goals 4. Provide on-going Care management support to assess progress and ensure treatment plan outcomes are met or changed as needed 5. Meet weekly with clients to provide therapeutic supportive Care management services addressing mental health, functional, and housing barriers as needed to help clients obtain and maintain housing 6. Conduct crisis and risk assessment, provide appropriate interventions as needed, and consult with supervisor, complete critical incident reporting with 24 hours in accordance with policies and contract requirements 7. Provide crisis intervention services focused on enhancing the clients' ability to independently problem solve, use effective coping skills, and manage and self-coordinate own care 8. Provide intentional Care management supports that move the client from a high intensive level of service to a more independent, recovery-focused level of service as appropriate 9. Use evidence-based practices in service delivery such as Recovery-Oriented Mental Health Services, Motivational Interviewing and Harm Reduction 10. Maintain documentation standards as set forth by the program contract and policies 11. Complete progress notes on every contact with client or collateral contact by the next business day

JOB QUALIFICATIONS

1. Bachelor's Degree in Social Work or a related field required 2. Three years of experience providing Care management services in a non-profit, government, or public service environment preferred 3. Maintain and execute confidential information per HIPAA standards 4. Homeless services, Care management, mental health, and permanent supportive housing experience preferred 5. One year OUD, STUD, other SUD experience required. 6. Flexible work schedule to include some evenings and/or weekends as needed 7. Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple Care management and health needs 8. Ability to work independently and within a team environment and exercise mature judgment 9. Ability to provide community and in-home services on a regular basis 10. Current Criminal Background Check required 11. Strong communication skills 12. Highly motivated self-starter with the ability to coordinate multiple projects/tasks simultaneously in a high pressure environment. 13. Basic CPR and AED certification required.

Pay:

$26.00 - $28.00 per hour Expected hours: 40.0 per week
