What You'll Do As a Rehabilitation Case Manager, you'll be the go-to person helping clients achieve greater independence.

You will:

Conduct comprehensive functional assessments and identify client needs Develop and coordinate service plans tailored to each client's goals Partner with instructional staff and the Division of Blind Services to deliver effective programs Monitor and celebrate client progress while keeping accurate records Provide community education and outreach to raise awareness of our mission Serve as an advocate and trusted resource for clients and their families Schedule Monday - Friday, 8:00 AM to 4:30 PM Onsite role What We're Looking For Bachelor's degree in Human Services or related field At least 2 years of experience in the field (experience with people with disabilities preferred) Strong communication, time management, and teamwork skills Comfort with Microsoft Office and online research A positive, motivated, and compassionate approach Requirements Ability to pass background checks Reliable transportation for community-based activities Valid Florida driver's license and required insurance coverage (if using personal vehicle for work travel) Physical Demands Ability to lift equipment used in program activities Why You'll Love Working Here We believe in supporting both our clients and our team.
