HAVE EXPERIENCE WITH DEVELOPMENTAL DISABILITIES 2. MUST HAVE WORKING KNOWLEDGE AS A SUPPORTED LIVING COACH OR CASE MANAGER. 3. MUST BE PROFICIENT WITH ACCURACY IN DOCUMENTATION DELIVERY. 4. MUST BE CERTIFIED AS A SUPPORTED LIVING COACH-NEXT COURSE IS FIRST WEEK IN AUGUST-2026

The Supported Living Coach plays a vital role in empowering families and individuals with developmental disabilities, including autism. This position focuses on providing guidance, support, and resources to enhance the quality of life for clients. The Supported Living Coach will work closely with clients in various settings, including home and community environments, to promote independence and personal growth. Duties Develop Implementation Plans tailored to the unique needs of each family and individual. Complete necessary paperwork including Housing Survey, Health and Safety and Quarterly Summaries. Assist Individuals with Training and Education on there Health Assist Individuals with attending there Medical appointments. Provide direct support to clients in managing daily activities and achieving personal goals. Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors. Collect and analyze data on client progress to inform adjustments in support strategies. Facilitate communication between clients, families, and other professionals involved in the care process. Assist families in navigating available resources and services within the community. Offer training and education to families on best practices for caregiving and support. Collaborate with interdisciplinary teams to ensure comprehensive care for clients. Skills Strong understanding of behavior management techniques. Experience working with individuals with developmental disabilities and Autsim Proficiency in direct support practices that foster independence and skill development. Excellent communication skills, both verbal and written, to effectively engage with clients and families. Be Proficient in computer skills and a quick learner Knowledge of special education principles and practices is beneficial. Ability to collect data systematically for tracking client progress. Experience in home community care settings is preferred. Background in caregiving or behavioral health is a plus or Case Manager experience is essential Associate or Bachelor's Degree preferred, Experience is great The Supported Living Coach position is essential for fostering a supportive environment where individuals can thrive.
