401(k) Dental insurance Paid time off Vision insurance We are seeking a highly skilled and organized Care Manager with a strong background in social work and /or RN, specializing in post-acute care and senior populations. This role is ideal for a professional who understands the complexities of care transitions, discharge planning, and ongoing support to older adults and their families. The Care Manager is to oversee and coordinate care services for our clients in a home care setting. The Care Manager will serve as the primary point of contact for clients and their families, ensuring personalized, high-quality care that promotes independence and well-being. Additional territories include Sebastian, Port St Lucie and Fort Pierce. Key Responsibilities (but not limited to): Access clients' needs through in-home evaluations and collaborate with families and healthcare providers. Develop and implement individualized care plans tailored to each client's preferences and medical requirements Assist in training and supervising care staff, including caregivers, home health aides and certified nursing assistants Monitor service delivery to ensure quality standards are met and adjust care plans as needed Act as a liaison between clients, families, healthcare professionals, third party provider partners and internal teams Maintain accurate documentation, including care plans, progress notes and compliance records Ensure compliance with all relevant regulations, licensing, and safety standards Provide ongoing support and education to clients and their families regarding care options and resources
Qualifications:
Ideal Candidate:
Minimum of 5 years of experience in home care, healthcare, or social services Knowledge of home care regulations, healthcare practices, and community resources Pass criminal background (Level 2 AHCA -https://info.flclearinghouse.com) Excellent interpersonal, communication, and organizational skills Ability to assess client needs and develop appropriate care plans Leadership skills and experience supervising staff Compassionate, patient, and dedicated to improving clients' quality of life
Certifications Required:
CPR Education:
