Job Summary The Housing Care Coordinator provides assessment, care planning, support, referral and ongoing care coordination to targeted individuals within the Permanent Supportive Housing program for chronically homeless individuals with disabilities. - Any other duties as assigned by management The role of the Housing Care Coordinator is clearly defined by a set of responsibilities which provide members and families with the best possible services available.
- Tracking and reporting of data, required outcomes and monitoring of standards for permanent supportive housing services
- Work with program leadership, landlords and community partners to support participants in maintaining housing
- Provide trauma informed, trauma responsive, participant specific support to assist individuals in maintaining a clean, safe, and habitable living environment.
- Participate in agency, provider, and statewide committees dealing with and /or related to individual/family services, housing services, mental health services and/or the development of these services for youth and families.
- Monitor agency outcomes, and assist staff in meeting, tracking and documenting agency outcomes.
- Enter data within required systems and support others to do so in a timely and accurate manner
- Refer clients to community resources or to specialists as necessary.
- Works as a liaison with collateral systems to resolve conflict, advocate for consumer rights and follow-up with specific problem outcomes
- Completes intake/ history/assessment and service plan for participants
- Routinely monitors and assesses progress and communicates outcomes, needs and barriers to the PSH team in an effective way
- Functions as a member of a team to ensure services for members, including case coverage as needed
- Advocates for participants with landlords and community resources.
- Assists members in maintaining benefits as needed
- Mediates conflicts related to member services and advocates for the member when needed
- Completes file audits and other quality assurance activities to ensure work meets requirements of the agency, state and regulatory bodies, and funding streams Supervisory Responsibilities The Housing Care Coordinator is directly responsible to and supervised by the Team Leader of Community-Based Services.
The Housing Care Coordinator does not directly supervise any staff. They collaborate with the other Community Based Services staff in order to meet the needs of all Community-Based Service recipients. Travel Requirements Travel is required as needed between business locations. Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience Bachelor's degree in psychology or social services field required; 1 year of experience providing mental health or homelessness services required; experience with care coordination preferred. Demonstrate a high level of detail orientation and organization. Possess good problem-solving skills and be able to follow through with assignments. Ability to read and interpret documentation, analyze and interpret data, use routine computer programs, to develop routine reports, and to handle non-routine crises. Certificates, Licenses, Registrations Licensure is not required for this position. Nurses must submit a copy of their current license.
