- The Social Worker
- LMSW provides professional social work services to patients and families to address psychosocial needs, support care transitions, and connect individuals to community resources.
- LMSW plays a key role in improving patient outcomes, reducing readmissions, and supporting vulnerable populations across the care continuum.
- Essential Functions
- + Conducts age-appropriate psychosocial assessments for patients across the lifespan, including neonatal, pediatric, adolescent, adult, and geriatric populations.
- Qualifications
- + Master's Degree in Social Work (MSW) or Psychology required + 2-4 years of experience in a healthcare or clinical social work setting preferred
- Knowledge, Skills and Abilities
- + Knowledge of psychosocial assessment techniques, clinical social work principles, and healthcare discharge planning.
- Licenses and Certifications
- + LMSW
- Licensed Medical Social Worker
- State Licensure required + BCLS
