Firefly Children & Family Alliance is currently seeking an Independent Living Case Manager! While this position is home-based, the primary office is in Indianapolis, IN. In this position, you will work with clients in Marion, Hamilton, and Hancock counties. You will collaborate with our internal treatment team and community agencies to directly advocate for at-risk children and families, providing important needs assessments, prevention and intervention services, and progress monitoring. This role requires the ability to adapt to a flexible work week, including evenings and potentially weekends.
You Will:
Direct Service - Direct contact, indirect contact, and brokered services (50%) Visit and assist each client in achieving their goals Broker support to clients Provide transportation to clients associated with specific goals Attend court with clients Respond to crisis (depending upon referral type) Develop relationships with agencies in the community Work with other social welfare agencies Refer clients to community agencies Report writing, information entry (50%) Email referring agent and supervisor following each contact or attempted contact Review and upload monthly reports to Kidtraks and NYTD Maintain case and agency paperwork according to standards Prepare written reports for the Department of Child Services / Probation office within identified time frames
You Have:
A bachelor's degree in social work or related field is required. You must be at least 21 years of age Work in-person, no remote work available for this position. A valid driver's license and insurance. Must not have 4 or more points on driving record. Knowledgeable regarding social service systems within the state Knowledge of and adherence to current confidentiality practices Appreciation for and ability to respond to cultural differences Maintain relevant networks of main contacts At least 1 year of home-based case management experience working with clients in Foster Care or transitioning out of Foster Care Lived experience in Foster Care is preferred, not required Ability to work a flexible schedule, including evenings and some weekends.
