Ideal Candidate:

Experienced Care Manager or Home Health Nurse with strong care coordination experience. Preferred Experience LTSS and Dual-Eligible populations Medicare/Medicaid managed care Population health or community health Behavioral health Complex care coordination Case Management Certification (CCM) preferred Day-to-Day Responsibilities This community-based Care Manager supports dual-eligible Medicare and Medicaid members with complex medical, behavioral, and social needs. The primary focus is conducting assessments to improve health outcomes and quality of life. This person will be completing 10 assessments per week (2 members per day).

Responsibilities include:

Conducting in-home and community-based member visits Completing comprehensive assessments, functional assessments and/or UCA Assessments UCA = Uniform Core Assessment UCA used to be referred to as an MDS assessment Additional Details Must reside within a commutable distance of Massachusetts/have a reliable vehicle 50%+ local travel required for member and provider visits Annual influenza vaccination required Community/mobile-based position with significant fieldwork throughout assigned territory Monday-Friday, 8am-5pm

Contract:

Through the end of the year (need commitment of work through the end of the year)

Pay:

$47.00 - $55.00 per hour
