We are currently hiring a Care Manager. As a Care Manager, you will assess & monitor the needs of the consumer & implement a service plan which is following the Executive Office of Elder Affairs & Elder Services of Cape Cod & the Islands Home Care Program regulations & contract obligations. This is a full-time position, Monday- Friday, 34 hours/ week plus unpaid one hour lunch each day. The pay rate for this position is \$30.46 per hour. What do you need to be considered for this position? Ability to collaborate, take initiative, provide/ receive feedback. Ability to communicate & work with the elderly & community resources serving the elderly population. Excellent computer skills required. Ability to work independently & professionally. Valid MA driver?s license & car required. Prolonged periods sitting at a desk & working on a computer. Must be able to lift up to 15 pounds. Travel Required daily throughout the service area.
